This 3rd supplementary statement was required when the NHS Trust suddenly produced a record of a key board meeting they previously denied the existence of (on the final day of a 16 day hearing)
These were the statements of my other witnesses including Jeremy Hunt, Sir Norman Lamb, my wife and the 2 expert consultant anaesthetists on the patient safety issues
This was application to strike out the NHS response to this case due to their poor conduct at the hearing
